Starting a PFCC Project

Six Quick Steps

Start a PFCC project today using the steps below:

  1. Select a PFCC Champion and leader(s), and preferably a caregiver
  2. Map your patient's experience and flow from beginning to end for the selected cycle of care
  3. Establish metrics and collect "Pre-project" information:
    • Internal questionnaires
    • Patient shadowing program
    • Flow and time studies
    • Staff input
    • Patient advisory council information
  4. Establish the PFCC Leadership Team including the PFCC Champion(s), the CEO/COO,Systems PFCC Senior Director, and Facilitator
  5. Establish the PFCC Working Group based on chosen project and patient flow studies
  6. The PFCC Working Group must meet weekly. Define list of projects and priorities as well as solutions based on metrics from #3
